Clinical Analysis of Surgical Treatment in Intrahepatic Stones



Abstract
Every stone in the biliary tree situated proximal to the origin of the common hepatic duct is considered intrahepatic lithiasis. This disease presents problems of dignosis and treatment on which there is still no uniform agreement.
This study is a clinical observation and results of surgical treatment of 73 cases of patients with intrahepatic stones, which represent 9.84% of 742 cases of patients with gallstones seen in the Department of Surgery, Chonnam University Hospital
from
January 1985 to December 1991.
@ES The results obtained were as follows:
@EN 1) The incidence of hepatolithiasis is 9.84% in biliary stones, and sex ratio of male to female was 1:1.1 and most prevalent age group was 6th decade.
2) intrahepatic stones were located in the left hepatic duct in 40 cases(54.8%), the right duct in 13 cases(17.8%) and both intrahepatic duct in 20 cases(27.4%).
3) Hepatectomy was done in 47.9%, non-hepatectomy was done in 52.1%. In the hepatectomy group, the good result was 71.4%, the poor result was 22.8%, and in the non-hepatectomy group, the good result was 36.8%, the poor result was 57.9%.
4) The remained stone was noted in 23.3% of operation cases. Postoperative complications were found 11 cases(15.1%) and most common was wound infection.
